About the Policy

A signed and completed application with payment is required in order for coverage to be issued. Complete all questions. Incomplete applications will be returned. Insure only one instrument per application.

On same side of the application, list bows, cases and any additional instrument pieces, and ALL parts of bell kits and drum sets in space provided. Enclose check or money order for full premium PAYABLE TO Music Agency, Inc.; or charge by VISA, MasterCard or DISCOVER.

If appraised value is used, attach appraisal. We recommend insuring instrument for replacement value.
